Care Coordinator - Southport And Formby Health

Description: Care Coordinator £21,892 to £24,157 a year dependant on experience | Permanent Full Time but Part Time will be considered Location: Southport Southport and Formby Health, is a GP Federation created to enhance health care services in the local area. We work closely with the local Primary Care Network to develop and deliver local services. We are looking to recruit a highly skilled and motivated individual to join our Enhanced Health in Care Homes team as a Care Coordinator. The team works to ensure that people living in Care Homes receive the same level of care that they would if they were living in their own home, and you will be a vital part of this team. The purpose of the Care Coordinator role is to be involved in the care and management of patients, supervising interdisciplinary care by bringing together the different specialists whose help the patient may need.

Requirements: You’ll need to have a good understanding of community health and social care systems, so this role would be ideal if you already work in an administrative role within a health care setting and are looking for a career development opportunity
